Hours of operation:
Monday through Wednesdays: 9:00 a.m. to 5:00 p.m., closed from noon to 1:00 p.m.
Monday and Wednesdays: patients seen by appointment only per SLCC medical provider
Tuesdays: walk-in clinic for patients already registered with the clinic
Thursdays: Diabetic clinic for existing patients with appointments 9:00 a.m. to 5:00 p.m.; open again at 5:00 to 6:30 p.m. for new patient registration and walk-in clinic (new patients successfully screened will see a volunteer physician.)
All new patients must be seen for the first time at Thursday night walk-in clinic. New patients must bring the following documentation so that they may be screened for eligibility and register with St. Luke Community Clinic:
✓ Proof of residency: A piece of U.S. Mail in its envelope delivered to patient's residence.
✓ Government-issued photo identification (i.e., driver's license, state-issued photo ID, passport).
✓ Proof of income: copies of one month's recent paycheck stubs and/or recent bank statement showing income received via direct deposit. If you receive food stamps, a copy of your current food stamp allocation letter.
✓ Federal tax return for most recently completed tax year. If none was filed, St. Luke Clinic may, at its discretion, assist patients in obtaining the necessary documentation from the Internal Revenue Service. However, ultimately it is the patient's responsibility to provide this documentation.
✓ Proof of application for Medicaid or "Denial of Medicaid Coverage" documentation; either must be dated within two years of your clinic visit. To apply for Medicaid in person, visit the Warren County Department of Social Services. To apply online, visit www.commonhelp.va.gov.
