Clinic Schedule  

Monday through Thursday:  9:00 a.m. to 5:00 p.m., closed from noon to 1:00 p.m.

Monday, Wednesday and Thursday:  Chronic care by appointment. 

Tuesday:  Walk-in clinic for established patients.

Thursday Evening: Walk-in clinic at 5:00–6:30 p.m. for new patient registration. All new patients must be seen for the first time at Thursday night walk-in clinic.

     •   New patients must bring the following documentation so that they may be screened for
          eligibility and registered with St. Luke Community Clinic:

✓ Proof of residency:  A piece of U.S. Mail in its envelope delivered to patient's residence.
   

✓ Government-issued photo identification (i.e., driver's license, state-issued photo ID, passport).

✓ Proof of income:  copies of one month's recent paycheck stubs and/or recent bank statement showing income received via direct deposit.  If you receive food stamps, a copy of your current food stamp allocation letter.

✓ Federal tax return for most recently completed tax year.  If none was filed, St. Luke Clinic may, at its discretion, assist patients in obtaining the necessary documentation from the Internal Revenue Service.  However, ultimately it is the patient's responsibility to provide this documentation. 

✓ Proof of application for Medicaid or "Denial of Medicaid Coverage" documentation; either must be dated within two years of your clinic visit.  To apply for Medicaid in person, visit the Warren County Department of Social Services.  To apply online, visit www.commonhelp.va.gov.

Friday:
  Closed, except for monthly women's health clinics by referral/appointment.

St. Luke Community Clinic is a nonprofit, community-based organization of volunteers and professional staff committed to providing high-quality health care services to low-income, uninsured residents of Front Royal and Warren County, VA.  St. Luke is a charitable-care clinic and charges a nominal fee for its services.  The clinic provides chronic and acute care and is not equipped to treat medical emergencies.
