St. Luke Community Clinic (SLCC) provides medical care and prescription medication for eligible adult Warren County, VA, residents with no health insurance. Total family income must be 250% or less of the Federal Poverty Level (FPL). Income sources include wages, child support payments, alimony, disability benefits, pension payments, TANF payments, and unemployment benefits. All patients are screened for eligibility upon their first visit and thereafter every six months.
For the clinic to determine eligibility, all patients must provide the following items:
✓ Proof of residency: A piece of U.S. Mail in its envelope delivered to patient's residence.
✓ Government-issued photo identification (i.e., driver's license, state-issued photo ID, passport)
✓ Proof of income: copies of one month's recent paycheck stubs and/or recent bank statement showing income received via direct deposit. If you receive food stamps, a copy of your current food stamp allocation letter.
✓ Federal tax return for most recently completed tax year. If none was filed, St. Luke Clinic may, at its discretion, assist patients in obtaining the necessary documentation from the Internal Revenue Service. However, ultimately it is the patient's responsibility to provide this documentation.
✓ Proof of application for Medicaid or "Denial of Medicaid Coverage" documentation; either must be dated within two years of your clinic visit. To apply for Medicaid in person, visit the Warren County Department of Social Services. To apply online, visit www.commonhelp.va.gov.
The clinic's nominal fees are as follows:
✓ $5.00 for initial eligibility screening and for each re-screening every six months.
✓ $5.00 before each visit with a nurse, nurse practitioner, physician assistant, or physician.
✓ $3.00 when patient receives medications from the clinic.
✓ $20.00, payable in advance, for dentistry appointments.
✓ $10.00 no-show fee when a patient does not cancel an appointment at least 24 hours in advance. No-show fees must be paid in full before future appointments will be scheduled.
